On line 45, the value for the givens field is "Helena", the suggested correction is "Helana". Submitted by Shelley. The following comment was attached: the 4th letter looks more like an "a" than an "e"
This correction was evaluated by Krystal Tait: I have checked the image of the original form and the correct value is debatable but I think the transcribed value is more likely to be correct.
