On line 22, the value for the givens field is "John Welsy(?)", the suggested correction is "John Welsey". Submitted by L.M. Bradley.
This correction was evaluated by J. Buller: I have checked the image of the original form and am very sure that the suggested correction does not reflect the original. This comment was added: Comparing the last letter to the ending of May on the next line, appears that the y may be slightly misshappen while trying to fit it into the space allowed.
