On line 10, the value for the givens field is "Frederick", the suggested correction is "Fredeick". Submitted by Shelley. The following comment was attached: magnification shows it to look like "Frederck, with the "r" dotted-could possibly be Fredrick, but the letter after the "d" doesn't quite look like an "r"
This correction was evaluated by Krystal Tait: A change other than the suggested correction has been made, I have checked the image of the original form and agree with the current value
