On line 28, the value for the givens field is "Elizah", the suggested correction is "Eliza N". Submitted by Gord Haddow.
This correction was evaluated by Joan Labbe: I have checked the image of the original form and the correct value is debatable but I think the transcribed value is more likely to be correct.
