On line 3, the value for the surname field is "Sides", the suggested correction is "Seiler". Submitted by Margaret Cox.
This correction was evaluated by Loraine Wilhelm: I have checked the image of the original form and the correct value is debatable but I think the transcribed value is more likely to be correct.
